Outreach, Linkage & Support Services
Every recovery journey is unique. The key to lasting change is having a full continuum of support – before, during and after treatment. By filling the gaps in outreach, linkage and support services, programs focus on enhancing successful engagement, entry in care, retention, and post-treatment services for people living with substance use disorders.
Programs
210 StreetReach
210 StreetReach is a street outreach team comprised of community health workers serving as a link for people using substances and local providers. The goal of the program is to build and foster relationships with agencies serving the substance-use population to better promote community access and retention in care.
210 StreetReach provides referrals, linkage, and support to people in need of substance use treatment, mental health services, medical care, and/or support services. This program also provides education and access to harm reduction supplies including Naloxone, bleach kits, smoke kits, condom kits, and hygiene kits.

Comprehensive Continuum of Care for Females (CCC)

CCC offers interim and linkage to support services to pregnant and parenting women diagnosed with substance use disorders (past or present) to help sustain recovery. CCC offers support from a Recovery Coach throughout the continuum of care (pre, during and post-treatment).
