Primary Care

Primary care is comprehensive medical care that addresses a person’s health and wellness. It focuses on the whole person – rather than a specific organ or disease. The goal of primary care is to prevent or solve any health-related issues over a period of time, and to coordinate the care a person receives.

For people living with mental illness or substance use, accessing primary care services can be challenging for a multitude of reasons. Maybe they feel the health care system has not met their needs, and they have given up hope that life can get better. They don’t access the medical care they so desperately need, and, as a result, end up dying 29 years earlier than the general population.

At the Center for Health Care Services, our highly skilled, compassionate team works tirelessly with people who don’t have the experience, resources or support necessary to navigate the traditional system, and may have given up on getting the help they need. We do this through a true, multidisciplinary, integrated-treatment team approach, recognizing first and foremost that the people we serve all have different healthcare needs, requiring different means of treatment.

True integration at its core, our team of, psychiatric providers, primary-care physicians, caseworkers and substance-abuse counselors all come together to manage comprehensive care based solely on what a consumer needs at exactly that moment – understanding that these needs don’t stay the same over time.
