Enrollment Screening Process

What to Bring:

  • Proof of Identification
  • Proof of Income
  • Insurance Information/Status
  • Proof of Residency
  • Proof of Guardianship
  • List of Medications

Once the registration process has been successfully completed, an intake screening will be provided on the same day or the next day.

What to Expect:

A clinician will complete a clinical assessment to determine a person’s eligibility for The Center for Health Care Services (CHCS) services. This process may take up to 3 hours. If he/she qualifies for services, the clinician will work with the individual to establish outpatient services at a CHCS clinic. If they do not qualify for services, the clinician will provide an appropriate referral in the community.
