The Center for Health Care Services (CHCS) is committed to protecting the confidentiality, integrity and security of our patients’ information and we regret to inform you of an incident involving some of that information.

On or about November 7, 2017, CHCS discovered that a former employee of CHCS had secretly taken personal health information from CHCS on a personal laptop computer. This discovery was made as a result of documents produced in litigation between the former employee and CHCS.

Among the protected health information taken includes up to the following: names, addresses, dates of birth, social security numbers, medical records numbers, dates of services, referral information, progress notes, types of services, diagnoses, medications, lab reports, treatment plans, discharge summaries, collateral hospital information, as well as, autopsy reports, toxicology reports, death summaries, and death certificates of deceased persons.

The information taken is believed to exist in the files of the former employee and his attorneys, as well as the attorneys for CHCS. There is no evidence to indicate that the information has been distributed outside of this group. Attorneys for CHCS have obtained a protective order to prevent further disclosure of the information, and to verify deletion of the information as soon as the court permits.

As a result of our discovery process during litigation, CHCS does not currently believe that the confidentiality of your information has been compromised; however, we are providing you this notice out of an abundance of caution. CHCS will provide further notification if the circumstances change materially.

We deeply regret any inconvenience this unfortunate incident may have caused our patients. CHCS has in place safeguards to ensure the privacy and security of all patient health information. Since this incident, CHCS has implemented additional measures to enhance the protection of patient information.
