The road to wellness is built on a partnership with the people we serve. The Center for Health Care Services Adult Behavioral Health Division offers comprehensive services in four clinics located throughout the city, including:

  • Psychiatric evaluation and treatment
  • Medical follow-ups and management
  • Wellness counseling and education
  • Individual and group therapy
  • Psychosocial rehabilitation
  • Case management
  • And other services to support each person’s goals for recovery

Assertive Community Treatment

The Assertive Community Treatment (ACT) program is a voluntary outpatient program designed for people who experience symptoms of severe and persistent mental illness resulting in frequent hospitalizations. A committed, compassionate, effective team helps people build skills to manage daily stressors and navigate life to overcome persistent psychiatric challenges that result in frequent hospitalizations or conflict.

Services include:

  • Psychiatric medication management
  • Medication training and support
  • Psychosocial rehabilitative services
  • Supported housing
  • 24/7 crisis services

Money Follows the Person (MFP)

MFP assists people discharged from care facilities who may need help transitioning to independence and life in the community. The MFP team helps with housing, coordination of Medicaid and STAR+PLUS services for support during discharge and transition, and provides connections to community resources. A team member continues to provide support after transition to the community and helps coordinate nursing or other assistance as needed.

Employment Connections

Finding a job is difficult, and the challenges of a mental illness can make it even harder to perform a job search, interview, and adjust to the routine of a daily job. The Center helps anyone enrolled in one of our programs develop resumes and interviewing skills and apply for a job. We work with local employers to identify jobs and provide job coaching to help clients obtain employment and start working.

Integrated Care Program

The Adult Behavioral Health Services Integrated Care team provides holistic medical, psychiatric and recovery services that empower patients to make positive life changes. The team works with each person to set and achieve goals for restoring and maintaining health and personal well-being. Services include medical treatment by psychiatric and primary care staff in the same clinic location, health and wellness coaching and support, therapy, skills development, and comprehensive coordination with community resources.

High Utilizer/Integrated Care Program

The High Utilizer/Integrated Care Program is a dedicated team of behavioral and primary care clinicians working together to provide comprehensive integrated health care for people living with serious mental illness, and an extensive history of emergency department utilization and/or inpatient hospitalizations for physical and/or mental health related reasons.
